An assessment is audit-defensible when an external reviewer can trace any score back to: the published rubric version it applied, the calibration state of the evaluator at the time, the observation it was based on, the finding it produced, the action assigned, the evidence of closure, and the outcome.

Most readiness records do not survive this chain. They were never built to. OCTAAR records are built to.
